The Office of the Public Advocate and the Office for Ageing Well have created the ‘Substitute Decision-Maker Toolkit’.

The aim of this project is to give Substitute Decision-Makers helpful tools and resources to carry out their role. It will also help Appointers to feel confident that the person they appoint as a Substitute Decision-Maker will be able to uphold their wishes.

The kit provides clear, consistent, and easy to understand information about Advance Care Directives and the important role of substitute decision-making.

As a part of this project, later in the year, the OPA will be out in the community delivering information sessions. At these information sessions, we will speak about the resources available, answer questions about substitute decision-making, and encourage people to write down their wishes, values, and preferences, for a time when they may need a Substitute Decision-Maker to make a decision for them.
